About the Population Health Toolkit

What is population health?

Population health is an approach aimed at improving the health of an entire population.  It is about improving the physical and mental health outcomes and wellbeing of people, whilst reducing health inequalities within and across a defined population. It includes action to reduce the occurrence of ill-health, including addressing wider determinants of health, and requires working with communities and partner agencies.

Why should we learn about population health?

It is essential for every healthcare organisation because population health principles have the potential to contribute significantly to improved health outcomes across the health and social care system.

COVID-19 has had a disproportionate impact on those already experiencing disadvantage, discrimination and health inequalities. The pandemic has highlighted the need for the NHS to increase the scale and pace of action to tackle health inequalities and protect those at greatest risk.

Taking a population health approach can help in the recovery from the pandemic and to improve patient access, experiences and the outcomes of those populations in most need.

Who needs to learn about population health?

This toolkit enables health and care organisations to embed population health approaches into their communities by improving the knowledge of their workforces in this area.

How can we learn about population health?

The Population Health Toolkit supports learners by providing links to free resources mapped to eleven core curriculum areas and learning objectives.

Learners may benefit from exploring the learning objectives to identify any gaps in their knowledge and visiting the relevant resources.

Accessing the Population Health Toolkit

This toolkit can be accessed by anyone who has an account for the elearning for healthcare (elfh) hub. If you do not have an account please register with any email address on the elfh hub. Once in the hub, use the search function to find and access the Population Health Toolkit.

The elearning programme will also be made available in the AICC and ESR (Electronic Staff Record) for the NHS health and care workforce.

How to Access

In order to access the Population Health Toolkit programme, you will need an elfh account. If you do not have one, then you can register by selecting the Register button below. Please note that you do not need to have an NHS email account to register.

To view the Population Health Toolkit programme, select the View button below. If you already have an account with e-LfH, you will be able to login and enrol on the programme from the View button.

You can also enrol on to the Population Health Toolkit programme by logging in to the e-LfH Hub, selecting My Account > Enrolment and selecting the programme. You can then access the programme immediately in the My e-Learning section.

NHS healthcare staff in England – ESR

The Population Health Toolkit programme is also available to NHS healthcare staff via the Electronic Staff Record (ESR). Accessing this e-learning via ESR means that your completions will transfer with you throughout your NHS career.
